The role provides clinical primary care delivered by a physician trained in comprehensive first contact and continuing care for patients with varied health concerns. The position will work closely with the clinical team and is primarily responsible for managing direct patient care, implementing evidence-based medicine, adhering to national and internal practice guidelines, providing patient advocacy, and driving clinical initiatives. /p h3You Will Focus On /h3 ul liHealth promotion /li liDisease prevention /li liHealth maintenance /li liCounseling /li liPatient education /li liAnnual Health Assessments (AHA) /li liChronic Care Management (CCM) /li liDirect and indirect patient care, identifying risks based on patient screening data /li liCollaboration with providers, ancillary personnel, community resources, and other interdisciplinary team members to resolve patient needs /li liUse of the Electronic Health Record (“EHR”) to manage and improve patient care, workflows and clinical compliance /li liPatient advocacy to achieve cost-effective care through coordination of healthcare services /li liPromoting effective communication with patients and encouraging the patient as a partner in health care /li /ul h3The Skills, Experience Education You Bring /h3 ul liProviding patient-centric care and serving as a patient health advocate /li liWorking in interdisciplinary teams and employing evidence-based practice /li liApplying quality improvement principles /li liAbility to function independently and as part of a team in a dynamic environment /li liExcellent communication skills with adaptability for patients, families, and colleagues /li liHighly organized, strong time management, and flexibility to manage multiple timelines and activities /li /ul h3Qualifications /h3 ul liMinimum of 1-2 years clinical experience /li liSuccessfully completed residency in their specialty /li liPreferred: 3+ years professional experience as a PCP /li liSignificant experience with an EHR, preferably Epic /li liActive and in good standing Medical License in California and Board Certified /li liActive NPI number /li liActive DEA number from US Drug Enforcement Administration /li liBoard certification must be current /li liCall coverage for assigned Brown Toland members 24/7, 365 days a year or participation in a call group /li /ul h3Compensation Benefits /h3 pThe base salary for this position is strong$225,000 - $255,000 per year /strong.
